Cleveland Room Request Form

Room availability is limited.

Room requests will be accepted only with a referral from a hospital social worker, case manager or coordinator, to ensure that families meet eligibility criteria.

A background check is required for ALL guests, including the patient, who are 18 years of age or older. You will be directed to the background check authorization form when you submit the room request form.
